The monopolar needle is a solid needle, insulated along its length except at the tip. It does not carry its reference: a surface electrode or a second needle is needed to close the circuit.

That montage changes what you see. The recording surface is larger than a concentric one and the pickup field wider: potentials look larger and a little less selective. Normative values are not the same, which is a reason not to mix the two types within one series of studies.

The main argument is tolerance. At equal diameter a monopolar is generally better tolerated, which counts in a child, in an anxious patient, or when the study has to explore several muscles.
